Application Process

The recent merger of Glasgow’s public housing and housing choice voucher programs has provided us with the opportunity to review various procedures to determine how we can make programs easier to access.

One of the first areas we are addressing is the housing and rental assistance application process.

You can now complete one Personal Declaration Form for both programs. This form is a “Pre-Application” form that can be printed from our web site.  Click here for a copy. 

Answer all questions on the form and bring it with you to your scheduled interview.  Remember, DO NOT sign the form until you come in for your appointment.  We must witness your signature.

If you are unable to print a copy of the Personal Declaration, you may also pick up a copy at the Glasgow office at 111 Bunche Avenue or the Cave City office at 200 E.P. Terry Estates.
